If you are a patient of an outside pain clinic looking to transfer care, please have your current pain provider or primary care provider send a referral. We do request that previous pain, procedural, and imaging records be requested whenever possible so we can provide you with the most appropriate treatment options. If you are already established with one of our providers when they were part of the Baxter Health pain department, you do not need to re-establish care. Depending on your insurance, you may need a new referral since your doctor has moved addresses. You may need to request your medical records be transferred to us.
